DFW Elective Procedures (the "Directory," "we," or "us") is an independent online directory of elective procedure providers serving the Dallas / Fort Worth metroplex. This policy explains what we collect, how we use it, and who we share it with.
What we collect
- Contact form submissions. Your name, email, phone number (if provided), procedure of interest, city, timing, and any optional message you write.
- Provider context. The provider, city, or procedure page you submitted the form from.
- Technical metadata. A hashed (not raw) form of your IP address for spam triage. We do not store the raw IP.
- Server logs. Our hosting provider may keep standard access logs (URL, user agent, timestamp). We do not analyze them for marketing.
We do not collect or store medical history, diagnosis information, insurance numbers, or other Protected Health Information (PHI). Please do not include sensitive medical details in the optional message field.
How we use it
- To route your contact details to providers in our directory that match your selected procedure and city.
- To respond to you if you contact us directly.
- To detect and prevent spam, abuse, or fraudulent submissions.
- To improve the site (aggregate analytics, no individual targeting).
Who we share with
- Matching providers. When you submit the contact form, we share your name, email, phone, procedure interest, city, and optional message with up to 5 providers that match your selection. These providers may then contact you directly.
- Service providers. Our hosting (Vercel) and database (Supabase) providers store the data on our behalf under their respective security and privacy policies.
- Legal compliance. If required by law, subpoena, or to protect rights and safety.
We do not sell your information to data brokers, lead resellers, or advertising networks. We do not run retargeting pixels.
Cookies and tracking
The site uses minimal cookies (session state and analytics). We do not use third-party advertising cookies or cross-site tracking.
Your choices
- Request deletion. Email hello@electivesurgerydfw.com with the email address you used to submit the form and we will remove your record within 14 days.
- Opt out of future routing. Same address as above.
- Texas residents. Under the Texas Data Privacy and Security Act (TDPSA), you may request access, correction, or deletion of personal data we maintain about you. We respond within 45 days.
Children
The directory is not intended for users under 18. We do not knowingly collect data from minors. If you believe we have, contact us and we will delete the record.
Security
We protect form submissions in transit (HTTPS) and at rest (managed Postgres on Supabase). No internet service can guarantee perfect security; if a breach occurs that affects your data we will notify you as required by applicable law.
Changes to this policy
If we change this policy materially, we will update the "Updated" date at the top and, where appropriate, post a notice on the site. The current version is always at https://electivesurgerydfw.com/privacy.
